To Rate a Child Care Provider:- Choose the type of provider you want to rate (a home or center based provider)
- Enter the zip code where the child care provider is located.
- Enter in part or all of the provider's name (Optional)
- From the search results, click on the provider you wish to rate. A review/rating form will appear on the left.
